b'GREATER MONADNOCK COMMUNITY HEALTH IMPROVEMENT PLAN 2015-2018 IV.COMMUNITY PRIORITY AREASThroughoutthedevelopmentofthisPlan,theCHCengageddiverseFactorSurveillanceSurvey,werenotalwaysavailableforthePublic HealthNetwork.Insuchcases,statisticsforCheshireCountywere community partners in a process to identify and evaluate current andused. emerging health issues, select regional public health priorities, and help planeffectiveinterventionsforthesepriorityareas.Throughthis community-based process, five health-related issues were identified as thepriorityhealthfocusareasfortheRegiontoaddressina coordinated manner over the next three years.These priorities are listed below in no specific order: \x01Behavioral Health \x01Substance and Alcohol Misuse \x01Tobacco \x01Obesity \x01Emergency Preparedness While these priority areas have been identified as most important for theRegiontoaddressatthistime,therearemanyotherhealth prioritiesfortheRegionincludingheartdiseaseandstroke,healthy mothers and babies, cancer prevention, asthma, injury prevention, and infectious disease, etc.Over time, it is the intent of the GMPHN and CHCtodevelopsimilarcontentfortheremainingpriorities,and potentially, identify new priorities. Thefollowingsectionsprovideanoverviewofeachregionalhealth priorityareaandidentifystrategicgoalsandobjectives.When possible, goals and objectives were aligned with existing regional and state initiatives such as Healthy Monadnock 2020 and the NH State HealthImprovementPlan.Communityleadersandsubjectmatter experts were consulted throughout the plan development process to ensure collaboration with existing initiatives and community partners.
