ThisCHIPwasdevelopedovertheperiodofFebruary2015to September2015fortheGreaterMonadnockPublicHealthNetwork Because a healthy community means more than good medical care,(GMPHN)RegionbytheCouncilforaHealthierCommunitywith this plan focuses on strategies that provide members of the Regions 33assistancefromtheSouthwestRegionPlanningCommission.The communities the opportunity to make healthy choices and the ability toGMPHN is one of 13 community health and safety networks statewide access health care when it is needed.With this in mind, the goals,that work to enhance public health-related services, improve regional objectives, and key actions proposed in this document are intended tocapacitytorespondtopublichealthemergencies,andtoconvene, coordinate, and facilitate public health partners. 6'
